*There is a $30.00 non-refundable activation fee for each "NEW" line setup with BYU–Hawaii.
The plans include unlimited talk, text, and data within the United States. The user of the line will be responsible for international and roaming charges. There is a $30.00 non-refundable activation fee for each new line.

Not Included in Plan
- International long distance fees and other miscellaneous charges are not included in the plan.
- Your line may be subject to suspension or cancellation if you are found making long distance calls out side of the U.S. or using your phone outside of the U.S. The student prepaid plan is for use within the U.S. while attending BYU–Hawaii.
SIM Card or eSIM Activation
An email will be sent to the email address provided on this request form when the SIM card is ready for pick-up or when the eSIM has been activated.

Added Information
- BYU–Hawaii Store is not responsible to setup your phone.
- BYU–Hawaii Store has provided helpful links on our website under FAQ.
- If your old phone is broken or lost and was not backed up to your cloud, you might not be able to get your information from your old phone.
- You are responsible to renew your prepaid plan each semester or term.
- BYU–Hawaii Store will send an email reminder 30 days before the end of your prepaid plan to renew.
- If your request to renew your line is not submitted by the 25th of the month that the prepaid plan ends, there will be a $15.00 late renewal fee from the 26th of the month to the end of the month that the prepaid plan ends in.
- If we do not receive a request to renew your line by the first of the new prepaid plan, your line will be cancelled and you will not be able to get your number back.
Non-Refundable Activation Fee
There is a $30.00 non-refundable activation fee for each "NEW" line setup with BYU–Hawaii.
If after submitting the request form you change your mind and decide to cancel the request, email cellphone@byuh.edu immediately to cancel the request form. Should you try to cancel the request form after a line has already been activated for the user, there will be no refund to the activation fee.
Late Fee to Renew Prepaid Plan
There is a $15.00 late fee if you renew your prepaid plan from the 26th of the month the plan ends to the last day of the month. (example: Winter prepaid plan ends on April 30th. If your renewal form is received between April 26th - 30th, there is a $15.00 late fee.)
Insurance
BYU–Hawaii Store does not provide insurance for your phone. The BYU–Hawaii Store is not responsible for your phone if it breaks or is lost.
Refunds or cancellations
The prepaid plan is not prorated, the plan price is set for the full semester or term. There are no refunds or adjustments if you sign-up after the start of the prepaid plan or cancel before the end of the prepaid plan.
Payment
All charges will be charged to your BYU-Hawaii Student account.
